Prime Insights: What inspired you to choose ophthalmology as your specialty, and what drives your passion for eye care today?
“What drives me is the moment a blind patient sees their grandchildren again after years—that joy is everything,” he shares.
Growing up in India, I saw the massive number of people affected by vision loss—especially due to cataracts, which are treatable.
Furthermore, my grandfather, who was a major inspiration in my life, always encouraged me to become a doctor and to help those less fortunate. It became clear to me that as an ophthalmologist, and an eye surgeon, I could fulfil both of these deeply held aspirations i.e. restoring sight and thereby changing lives.
What drives my passion for eye care today is the profound impact I see through my work, especially in the numerous camps and my involvement with trusts and NGOs. I see patients who have been blind for years, seeing their grandchildren again for the first time after surgery. That moment of joy, that spark in their eyes, is what keeps me going.
Knowing that many individuals in India cannot afford quality cataract surgery due to financial constraint, and it motivates me to perform these surgeries for free. Operating on these patients and seeing their immediate joy as their vision returns—that is what truly fuels me to do better every time. It is the profound satisfaction of making a real difference in their lives and society.
Prime Insights: How do you ensure precision and reliability in diagnosing and treating complex eye conditions?
We invest in top-tier technology with state of art infrastructure—slit lamps, microscopes, lasers, and sophisticated surgery systems. But technology alone is not enough. We have a highly trained staff that supports accurate diagnosis and treatment. All these elements combined enable us to maintain precision, achieve accurate diagnoses and provide effective treatmentin every case.
Prime Insights: In your view, what distinguishes your approach or practice from other ophthalmologists in the country?
“I never push unnecessary procedures. Patients deserve transparency and choices,” he asserts.
I do not view patients through a commercial lens. My work with trusts and NGOs has shaped a practice that is rooted in compassion, not profit. Every patient gets personalized,treatment tailored to their specific needs. I aim to provide comprehensive eye care with a compassionate approach andensure that this care is affordable for them.
Prime Insights: How do you keep abreast of the latest advancements in ophthalmology and eye care?
I make it a priority to attend numerous conferences, both nationally and internationally. I am actively involved in presenting papers at these conferences and contribute by writing papers for various national and international publications. Sharing the stage and exchanging ideas with other ophthalmologists during conferences and workshops is invaluable. This continuous interaction with colleagues and experts in the field helps me stay updated on the latest technologies and advancements in ophthalmology and eye care and bring the best global practices to my patients.
Prime Insights: Can you share the story of a challenging case and how you successfully managed it?
I have operated on many patients over the age of 100—even up to 108! Restoring vision at that age is deeply satisfying. Seeing the happiness, especially in those who were blind and regained vision, is incredibly rewarding. It is hard to single out one patient, as each case teaches me something and motivates me to improve.Every case brings a lesson and reminds me why this work matters.
We are also heavily focused on educating people about glaucoma, also called “Silent thief of vision” as it mayprogressvery slowly, Significant vision loss usually occurswithout patient’s realization and unfortunately, the loss is irreversible. Early diagnosis and treatment are crucial to prevent permanent blindness.
Similarly, with diabetic retinopathy, a major concern given India’s status as the diabetes capital of the world, patients often do not notice issues until irreversible damage has occurred. Prevention through timely detection and management of diabetes and its impact on the eyes is paramount. For both glaucoma and diabetic retinopathy, our approach encompasses early detection.Early intervention and timely management are key in preventing vision loss from glaucoma and diabetic retinopathy.
Prime Insights: Eye care often involves building trust with patients. How do you ensure a compassionate and empathetic approach in your practice?
Building trust starts with clear communication. Most of the people coming to me are scared of unknown and I always start by calming their fears. I would never start my consultation by emphasizing on what is lost or overwhelming the patient with negatives, which would only increase the fear and shut down the patient’s willingness to listen. I start byhighlighting the hope and assure the patient of saving the vision that remains and work towards improving and restoring the lost vision. I explain everything in simple language, like teaching a child.I believe in taking the time to educate each patient about their disease, for instance, explaining what disease is and how it can harm the body. Understanding the gravity of the situation is crucial for patient compliance with treatment.
Every patient is important for me, and I aim for them to leave with a clear understanding of their disease process. Patient education is paramount and builds trust. I want each patient to walk out informed and empowered.
Prime Insights: How do you manage your time effectively between patient consultations, surgeries, administrative duties, and professional development?
I am a very methodical person. Everything is pre-planned—my surgeries, personal time, even vacations. For instance, if I need a week-long holiday, I plan it at least six months in advance, arranging for alternate doctors and surgeons to cover my absence from my four hospitals. I am the chief surgeon and director, operating in all centres, so planning is non-negotiable, and it helps me inmanaging my time effectively and ensure smooth operations across all my commitments. Without it, chaos would follow.
Prime Insights: How do you incorporate innovation into diagnosis, treatment planning, and surgical interventions?
I incorporate innovation by staying closely informed about the latest technologies in ophthalmology. If I believe that a recently introduced technology or technique can genuinely benefit my patients, I try to integrate it into my diagnostic, treatment planning, and surgical approaches. My primary focus is always on providing the best possible outcome for those under my care.
Prime Insights: Have you been involved in community outreach, academic teaching, or public awareness programs for eye health? If so, please elaborate.
I am associated with the Ayushman Bharat scheme of the Government of India. I am also involved with the District Blindness Control Program. Through my trust in the memory of my grandfather, Shree Jai Prakash Agrawal Charitable Trust,we offer free surgeries to those in need. These initiatives are a core part of my work.
Prime Insights: What measures do you take to ensure ethical standards and transparency in your practice?
My approach to ethical standards is deeply influenced by my grandfather’s teachings, to always be honest and ethical. I believe earning trust is far more valuable than anything else.
I strive to avoid unethical practices, which I believe is why I have many returning and happy patients. Ethical conduct stems from one’s principles and upbringing.
Often, different doctors can have valid, differing opinions and approach to a treatment. Some might confidently recommend surgery for quicker healing, while others prefer a more conservative “wait and see” approach. It is crucial to give patients options and explain the pros and cons of each, empowering them to make informed decisions.
Prime Insights: Could you share any awards, recognitions, or significant milestones that highlight your contributions to ophthalmology?
Being recognized as one of India’s fastest cataract surgeons—performing over 18 surgeries per hour, and more than 100 surgeries in a single day, multiple times is satisfying in itself. But the greatest reward is the positive impact, I have on my patients’ vision by striving for improving their quality of life, in every surgery I perform.
As a visiting surgeon in Africa, I have the privilege of helping some of the poorest patients regain their sight. Even small improvements in their vision can profoundly impact their daily lives and restore hope.
Prime Insights: How do you cope with the emotional and psychological stress of managing a critical or life-altering diagnosis?
Meditation helps. It is a practice that helps me keep calm even in high pressure situations.Additionally, I have a strong support system in my children and my wife, who is also an ophthalmologist and understands the pressures of the profession. Their support, along with that of my parents, my brother and my friends, is invaluable inkeeping me grounded.
Prime Insights: What advice would you give to young medical professionals aiming to excel in ophthalmology?
His message to young doctors is clear:
“First, be a good human. Then, a good clinician. Only then can you be a great doctor. Compassion must lead your practice.”
My advice to those entering ophthalmology is this: first, strive to be a good human being. Only then focus on becoming a knowledgeable clinician, and later, a skilled surgeon. In my view, this is the true path to becoming a good doctor. Above all, never lose sight of empathy and compassion—these are the qualities that will define your practice and your impact
Prime Insights: What is your vision for the future of eye care in India, and how do you envision contributing to its advancement?
My dream is for everyone to achieve 6/6 vision. That is why I focus on free camps, government programs, and preventive care. We are striving for a future where no one loses sight due to lack of access.
